Application Forms

NEW MEMBERS: If your child hasn't played for NSB PAL in the past two years, please fill out a new Member Application which is below.  If you are able to print out and fill out, please bring to the New Smyrna Beach Police Dept., 1400 N. Dixie Freeway, NSB (beside the NSB Airport) to the south end of the bldg, (Records Dept.) and ask for Doris Mills.  We can notarize here at the PD and make a copy of the birth certificate, which is also needed.

Once these documents are on file, they are good for 1 - 2 years.

Registration forms for individual sports are provided on the FORMS link.  If you cannot print out any of these forms, come to the location above and we will provide.
